Medication errors are a major cause of preventable harm in healthcare, yet they often go unreported by nursing students during clinical placements due to fear of academic consequences and a lack of psychological safety. This project explores how clinical nursing instructors influence the culture of safety and medication incident reporting within undergraduate education. Using a mixed-methods approach, the research will examine the barriers and supports affecting students’ willingness to report medication incidents and provide actionable recommendations to improve teaching practices, support systems, and reporting frameworks in clinical education settings.
This research will inform the development of safer, more supportive clinical learning environments where nursing students are empowered to report medication incidents without fear. Findings will contribute to national safety goals by strengthening nursing education, reducing medication errors, and fostering a culture of accountability and continuous improvement.
