Venous thromboembolism (VTE) is a leading cause of death in cancer patients and contributes to delayed treatments, reduced quality of life, and increased healthcare costs. Despite strong evidence supporting the use of low-dose anticoagulation to prevent VTE, uptake remains low in real-world outpatient settings. This project will implement a multidisciplinary VTE prevention program at the Windsor Regional Cancer Program to identify high-risk patients using a validated risk score and provide education and personalized thromboprophylaxis plans. Through a pre- and post-implementation study design, the project will assess the feasibility and impact of the program on prescribing patterns, adherence, patient education, and outcomes. The program will also evaluate patient perspectives to guide future improvements. By closing the gap between guidelines and practice, this initiative aims to reduce VTE events in a high-risk population.
This project will demonstrate the feasibility of implementing a team-based thromboprophylaxis program in a community cancer centre, helping to prevent life-threatening blood clots in patients undergoing chemotherapy. The goal is to inform broader adoption of similar programs across other outpatient cancer settings.
