Community partners and academic researchers are working together to evaluate a housing-based model that helps socially vulnerable adults access primary care. Following early success in connecting 31 tenants to a primary care provider, the study will refine and test the approach with the goal of improving healthcare access and reducing barriers to care.

Together, these activities will help determine how housing-based outreach can improve access to primary care, strengthen early tenancy support, and promote better health outcomes for structurally vulnerable tenants in Windsor-Essex.
